The Health Science Index uses a unique method of inverted weighted rank order to help patients and care providers with patient monitoring. The app is able to surface the determinants of health which a particular patient should focus on the most. It also uses this normalized stack ranked assessment data to monitor patient progress.

The solution identifies the first things to work on and re-evaluates needs on an on-going basis – without the need for ad-hoc surveys or complex reporting inputs.

By employing a “living survey” and dynamic prioritization, users can identify change in a more timely manner with much less overhead associated with current state assessment.
